Please click on the form that you need and print it.  Then, fill it out and return it to your coach. 

Athletic Permission Form - Only one form per student is needed for the entire school year.  Please include all sports that your student might participate in for that year. 

Sports Physical Form - All sports physicals are considered good for 13 months.  Students who are not current on their physical will not be allowed to participate in practice until they get their physical.

Sports Camp Form - This form is necessary for students in order to participate in any camp offered through Young Americans Christian School.
